Medication Reconciliation Technician (Pharmacy Tech)

CompleteRx has an opportunity for a full-time Medication Reconciliation Technician to provide patient care at a 96-bed acute care facility located in Greenville, Texas.

As the Medication Reconciliation Technician, you will work 100% in the emergency room, interacting with patients, nurses, and doctors. The individual will speak with patients to gather information regarding their medicines, contact the pharmacy or physician's office to verify medication history, and enter it into the system.

Position Summary:

Under the supervision of a pharmacist, the Medication Reconciliation Technician will obtain (or review) admitting medication histories and reconcile home medications with medications ordered upon admission. The medication reconciliation technician will assist with quality improvement initiatives and reports.

What You Will Do

  • Interview patients to obtain or verify medication histories.
  • Contact retail pharmacies and physician offices to complete or verify medication histories.
  • Enter home medication lists into the appropriate computer system or paper form.
  • Demonstrates proficient skills in using computer systems and paper charts for data retrieval.
  • Assists with quality improvement reports and programs.
  • Displays good decision-making skills.
  • Works closely with the staff pharmacist on duty.
  • Promptly brings clarification/intervention issues to the pharmacist’s attention for resolution.
  • Communicates effectively with other healthcare professionals and patients.
  • Complies with all state and federal pharmacy regulations and requirements.
  • Demonstrates knowledge and competency in providing for needs of the population served.
  • Maintains continuing education/professional growth and development.

What You Will Need

Requirements:

  • Possess current pharmacy technician license in the State of Texas required.
  • Possess and maintains an active National Pharmacy Technician Certification (CPhT) required.
  • Two (2) years prior retail pharmacy technician experience required; or
  • Five (5) years prior hospital pharmacy technician experience required; or
  • Four (4) years prior hospital pharmacy technician experience with an ASHP Medication History-Taking Certificate required.
  • ASHP Medication History-Taking Certificate required.
  • Basic college courses; post high school vocation/specialized training preferred.
  • Licensed practical nurse experience desirable.
